mine was simple because my tongue piercing was 7/16"(11.1mm) before i had it cut, made for easy business. no forceps were used. it was marked, and i just held the left side while MontanaPiercer held the right side, then he slipped the scalpel in the hole and followed the mark. i did bleed of course, but compared to what i have seen it wasn't bad at all, and he also said it wasn't as bad as what he has seen. i had it cauterized then sutured, but the sutures were only put in because i insisted on them. i later discovered they were pointless and more of a hassle than i thought they might be.

having a stretched tongue piercing for a while also helps with the issue of regrowth once you have your tongue split. i have experienced no regrowth what so ever because of the size of my old tongue piercing, when i have read that others have had to go get it split again/further because of regrowth issues when they had no tongue piercing or one at a smaller size. plus it is just easier for the person performing the split methinks. i'm going to likely have mine split further in the future (after i have my tongue webbing cut and stretch another future piercing a bit) anyways, but i want a really long split. i can't really control each side separately either, and i know that has something to do with the fact that the split just isn't long enough for that just now, but mine is at a decent length, i just always like to perfect my modifications haha.

I had my tongue scalpelled to 4ga and after about 6 months, I took a scalpel and cut forward. Very little blood, very little pain, healed quick, was eating an hour later.
I had my tongue webbing cut a couple months after that and had it cut and cauterized. There was a lot of regrowth. I got it pierced at 8ga, and cut back to that (DIY) and there was little regrowth.
Some artists charge around $300-450 for it.. at least, in Toronto.

Mine cost 75 pounds I have no idea why it was so cheap
