12 gauge or 12mm? both are units, neither is a physical quantity

sorry, that was a stupid question, a barbell with 12mm girth would be extremely heavy and the spheres would be quite a bother for your tighs, you might want to try custom jewellery, maybe a hollow 12mm thick body with smartie beads on both ends? even if you still want a more traditional looking barbell, the way would be to contact companies that make custom jewellery

one tip: i'd most certainly go with titanium, even though it's going to be a lot more expensive, the lower weight it'll be on your precious will certainly justify the price

I've seen some rather large gauge genital piercings on males, and they don't seem to cause issues, other than occasional funny walking due to a large number of piercings.. If the weight is an issue, which I don't think it generally is, a material like niobium, which is a little lighter, might be preferable. Either way, custom made will probably be necessary. I'd recommend speaking with a piercer who can make sure it's the exact size you need. Otherwise, perhaps try bodyartforms.com.
