If I'm not wrong your idea is a Transcrotal.  A hole from the front to the back of the scrotum.
 
The same procedure must be used because, as I said, risks are important if you only pierce the scrotum and place a barbell.
You need to cut and suture.
Most of informations I had about the size of the cut were 8-10mm large. As I said, we deceided to cut at 10mm...and it was not to much ! Because the tissue inflate a bit after the cut and it does not help for the suture.

Hi ! I did a similar "Mod" some months ago.
I wanted a "deep scrotal piercing" and asked their point of view to 2 'bodmoders' (my regular, and Lukas Zpira which only is a contact ;) ).
 
Given that I wanted to pierce 40mm of tissue (1.5inch or something like that, sorry I'm french and I don't know exact conversion) they answered me that a Cut+suture was the better solution to avoid risks given that this procedure is similar to a Transcrotal.
So, due to sutures, the final length of the 'piercing' is 20mm (mathematical logic :) ).
 
So we did it like that, it was a pretty long procedure...
 
We used custom 10mm PTFE plugs (various lengths) with silicone O-rings during 3 weeks or something like that.
Next, when I had a better idea of the right length to use, I ordered a custom Titanium flesh tube (I don't know if I'm authorized to name the jewelry manufacturer who made it...if someone need it I can give the address. But I think that it is easy to find on the web).
 
Firsts steps of the healing process were relatively easy (compared to my Ampallang, Guiches or Hafadas... less lymph than Hafadas for example). After 3 weeks all was "okay", "closed up" if we can say it like that...I'm not sure ;).
 
Now, more than 5 months after the procedure, due to natural movements of scrotal tissue the 'piercing' is sometime a bit "red", perhaps we can call it "irritated" but never troublesome (one more time I'm not sure if the word is correct ;) ) or painful.
So I will wait several months and begin to stretch it :->.
 
It is as risky as a 'regular' Transcrotal because it is the same procedure and it cuts same internal tissues. I will not explain various risks, all informations are on the web ;)...
So it must be done by an experienced bodmoder (or DIY by someone who is not afraid by the loss of his balls ;) ).

Edit : Pay attention to the jewelry (size, material, length, weight, O-ring on Plugs, etc) because the scrotal tissue is very elastic !
Two times, my plug falled down when I was at work :-/

For something as deep as that, being closer to a transscrotal, I'd definitely say have it scalpelled and sutured together.
A transscrotal shouldn't be done with a needle, it'll never heal.

If you want a deep lorum at a similar size to that picture i reckon you have 2 choices.
1 - pierce it, insert a barbell and stretch over time. I don't think this placement would have half the issues of a transcrotal.

don't do this with a just a piercing. I had a transcrotal piercing and all it did was ooze. Healing time for a piercing like this would be about 2 years if at all. you'll want a longer barbell during healing because of swelling. in that area, a 1/4" barbell is much too small. for any piercing it is generally too small. if you want to have something that is a custom length, try PTFE rod. it can be cut down to the desired size you want and can be worn during healing. here's a page selling some in different gauges:

you will need to purchase 10g surgical steel beads to thread on to the rod though. if you want to just go with steel or any other material or metal, check out bodyartforms.com. they have a lot of stuff in different lengths and styles, as well as price ranges.

i don't understand what you are saying about the skin layers. do you mean you want them to be closer together to create a better fistula and improve healing for future stretching? i've never heard of anyone think that way, but in my opinion, it won't really work. you shouldn't have any problems healing the piercing as is, you don't have to try and keep the skin together, because if you tried to with a small barbell, the swelling and irritation from its small size would bother the piercing and delay healing, the piercing can't breathe with a small barbell. that may also lead to migration, rejection, and maybe infections.

the o-ring idea isn't a good one because the materials shouldn't be put near a healing piercing, plus i've heard instances of them even embedding into the healing skin and of course there's a larger possibility of them falling off than beads and the jewellery may fall out altogether. going back to the layers of skin healing together, i don't think i've ever heard of it happening with just a piercing. surgically it could happen, but i don't think with a piercing, and keeping jewellery too tight to the skin goes back to irritation, infections and migration or rejection.

if you can find other artists to chat with online, that would be a good idea. i'd recommend BME and IAM but as of right now, they aren't in control by a person i trust. most doctors probably won't give you any good advice anyways, they aren't usually knowledgeable about body modification, and that is understandable, they aren't really trained to be. often times they will just tell you not to, and to avoid the possibility of complications to not go through with it at all. a piercer is your best bet for any good advice.

but i think you are just thinking about this too much. you could pierce the area and just wear jewellery of the appropriate length. if deep enough, you could accomplish a good amount of stretching. some people do have piercings like that scalpelled larger, but if you do not want to go through with that, i say you purchase some jewellery and needles and go for the piercing itself, once you feel ready.

www.painfulpleasures.com sells needles and piercing supplies in various sizes. the piercing at 14g and tapering up to 10g would be kind of annoying during healing i would assume. the pain wouldn't be extreme, but there would be a lot of swelling, and the trauma from tapering larger would delay healing more. it isn't something i would do. check out the site i just gave and see if you are interested in purchasing needles and doing it yourself or not.

I'm collecting all information I can get before to do something. But it's so difficult here in Brazil.

Genital piercings are very uncommon here. Every time I tell to someone that I have a genital piercing, the people stare at me as if I am some kind of scaled alien, with 5 eyes and 3 noses. :-)

Doctors dislike piercings of any kind. If I talk about it with a brazilian doctor, I will not get support or information - only reproving.

Here, piercing studios have a large experience in navel and facial piercing, nothing more. They only do piercings using a 14 ga catheter, needles in large gauge you can get it nowhere in Brazil. When I decided to get my PA with 10ga, one of the best studios in São Paulo (my city) told me that they would use a 14 ga catheter followed by two tappers, to reach the desired 10 ga (How much would it hurt?).

The internet is the only way I can get some information. In the meanwhile, I will be doing my ladder. :-)

Yeah, "deep lorum" is a good name to what I want.

I thought that using o-rings would be a good manner to fix the two skin layers together, not allowing they go aside and forming a long fistula and making easy to clean swelling and some discharge. But if itsn't a good idea... no o-rings.

Once there is only 1/8" of skin in the place i want to pierce, do you think a 1/4" barbell would be OK?
Would´t the movement of the skin cause the balls be buried? Is there some problems in trying to make the two two skin layers together?

what´s your opinion?

Allowing any air or other substance into a piercing can cause infection. 

I can't give you all your answers but I can say this:
Just like with all mods, be careful.  This is an especially sensitive spot when in comes to healing (and in general it's sensitive).  Get a hold of someone who did this successfully and someone who did this unsuccessfully and see what there issues were.  Contact piercers and hear their input. 
I know you don't want to use a surgeon but often doctors will be nice enough to tell you their input even if you don't use their services.  That would help too.

i wouldn't recommend o-rings during healing. i'd say a straight barbell of the appropriate length should do fine.

as for placement, i thought you might be talking about doing a transscrotal piercing, which is placed in between both testicles. it is a dangerous piercing that can take forever to heal, or not even heal at all, and can harbour nasty infections since it isn't all skin in the scrotum. the placement you would want is more like a lorum piercing, and i don't see the problem with a deeper one. of course you should do more research on such piercing and the anatomy before you attempt, but i think what you want is possible.

Second, my question:

I'm thinkink about a scrotal piercing similar to the photo at the bottom of this message, but I don't want to do it surgically.

I'm thinking if could I do the piercing with a 10 ga needle and wear a bar with o'rings, to make the two

sides of he scrotum skin stay togheter. If the two sides heal togheter and form a short fistula from side

to side, I could stretch it after healing.

Would it work, perhaps?

I heard that deep scrotal piercings never heal, that alloW air and water to enter in the internal scrotal

sac causing infections, that your nuts will rot and a lot more.

I'm not sure what is real and what isn't.

How deep a scrotal piercing can be?

Is there somebody with some experience in scrotal piercings?
